Jade Scott was last seen Christmas Day at her home in the 200 block of Cruise Street.
Police said her caregivers are concerned because she may be in a vulnerable state.
She is described as being five feet seven inches tall, 130 pounds, with short brown hair, brown eyes and pierced ears.
She was last seen wearing a black toque, black pants, and a beige sweater.
